Please download the Chiropractic New Patient Paperwork, print, and complete it before your initial appointment. If you are unable to do so, please arrive 15 minutes prior to your scheduled appointment time to complete the necessary paperwork. We know your time is valuable so every attempt is made to run on time.

Please wear or bring athletic type clothing to your initial appointment. It is not fully necessary but will facilitate the initial examination process. Your initial appointment will last from 30-60 minutes depending on the complexity of your case.

Health Optimization Patients

Please download the Health Optimization New Patient Paperwork. This paperwork must be completed and returned to the clinic a minimum 3 days prior to your initial appointment. Health Optimization appointments must be scheduled 7 – 14 days ahead of time. The paperwork is quite lengthy and requires you to complete a 3-day food journal. Please give yourself adequate time to complete it.
